The Registration Form is required for infants through kindergarten checked into the nursery during Sunday morning worship.
The Web/Photo Release Form is required for your child's pictures to be placed on our nursery page or other nursery publications. Fill out, print, sign, and return to the nursery information desk.
The Emergency Contact/Medical Release Form is required for activities that are held off-site, such as Parents Night Out.
This form is to be completed and submitted to Alan Heath in the church office before working or volunteering in our nursery.
